Guidelines to Completing Form
- Submit Suitability Acknowledgement or Consumer Profile Information form with every annuity application and with every life insurance application when replacing an annuity contract
- Complete the entire form
- Consumer must initial and date any changes
- Complete the form based on the proposed owner’s information except if the proposed owner is:
- a trust, corporation, partnership, or other non- natural entity; or
- set up under the Uniform Transfer to Minors Act
or the Uniform Gift to Minors Act
- Follow instructions on form; refer to Agent’s Guide for detailed instructions
