Suitability Acknowledgement / Consumer Profile Information Form
Before we move on to the specifics about Assurity’s annuity products, let’s go over a few helpful guidelines for completing the Suitability Acknowledgment or Consumer Profile Information form.
As mentioned, the agent must submit the applicable form with every annuity application and with every life insurance application when replacing any annuity contract. Make sure you complete the entire form, because conflicting or missing information could delay processing the application. Have the consumer initial and date any changes.
If the proposed owner is not the proposed annuitant, complete the form based on the proposed owner’s information, unless the proposed owner is a trust, corporation, partnership, or other non-natural entity. Otherwise, set it up under the Uniform Transfer to Minors Act or the Uniform Gift to Minors Act.

Guidelines to Completing Form
- Submit Suitability Acknowledgement or Consumer Profile Information form with every annuity application and with every life insurance application when replacing an annuity contract
- Complete the entire form
- Consumer must initial and date any changes
- Complete the form based on the proposed owner’s information except if the proposed owner is:
- a trust, corporation, partnership, or other non-natural entity; or
- set up under the Uniform Transfer to Minors Act or the Uniform Gift to Minors Act
